**Child Age 0-4 Years:**

No longer accepting new patients.

**Child Age 5-18 Years:**

$59/month with membership of parent

**Adult Age 19-39 Years:**

$99/month

**Adult Age 40-69 Years:**

$119/month

**Adult Age 70-99 Years:**

$129/month

**Adult Age 100+ Years:**

FREE

**Family Monthly Maximum:**

$325/month (2 adults + children under age 19)

Enrollment fee: $150 (charged after your first visit)

Young Adult/Student discounts available upon request (through age 26)

**No commitments. Cancel anytime.**
 A $400 re-initiation fee per person will be charged if you choose to join us again after a cancellation.
There is a $50 fee for no-show appointments.
