Child Age 0-18 Years:

$49/month with membership of parent

Adult Age 19-39 Years:

$89/month

Adult Age 40-64 Years:

$99/month

Adult Age 65+ Years:

$109/month

Family Monthly Maximum:

$250/month (2 adults + children)

Enrollment fee: $150 (charged after your first visit)

Young Adult/Student discounts available upon request (to age 26)

No commitments. Cancel anytime.

A $400 re-initiation fee per person will be charged if you choose to join us again after a cancellation.

There is a $50 fee for no-show appointments.

Altucare is currently accepting patients off a waitlist.

Concierge Medical Consultations are available on a case by case basis.
